To Register for a clinic, either download & complete the spreadsheet form or print out the PDF form. You can email the completed spreadsheet to the association, or print & fax, or print & mail, or print & dropoff to the NMHA Offices now at the Magna Center.

Payment must be received prior to clinic date.
For clinics with ice session must bring your helmet, skates, gloves and hockey stick.
Candidates must attend the entire program; latecomers may not be accepted.
If attending a Coach Refresher clinic, please indicate your current level of certification for renewal.
All team officials must be PRS certified. PRS can also be taken online thru the OMHA website www.omha.net.
